Waiver
RELEASE, INDEMNITY, WARRANTY REGISTRATION AND ASSUMPTION OF RISK
In consideration of your acceptance of this entry to the above event hosted by the Moose Jaw Koseikan Judo Club, I for myself, contestant, my heirs, executors, administrators and assigns, do hereby release and discharge the Saskatchewan Kodokan Black Belt Association (“Judo Saskatchewan”), Moose Jaw Koseikan Judo Club, City of Moose Jaw, and the officers, directors, employees, volunteers and any and all representatives of each organization from any and all claims or demands, losses, or injuries incurred or sustained by me (or parents/guardians if contestant is less than 18 yrs) as a result of attending, practising for or travelling to or from the event held in Moose Jaw on July 18, 2026.
As part of our camp activities, photographs and video recordings may be taken during classes, games, demonstrations, meals, and other camp events. By signing this form, I grant permission to the camp organizers, coaches, volunteers, and their authorized representatives to photograph and/or record the participant named above.
I understand that these photographs and recordings may be used for the following purposes:
- Promoting future camps and club programs.
- The club's website and social media pages.
- Newsletters, brochures, posters, and other promotional materials.
- Local newspapers or media coverage of camp activities.
I understand that:
- No compensation will be provided for the use of these photographs or recordings.
- Images may be used without identifying the participant by name whenever reasonably possible.
- Once images are published online or in printed materials, they may not be able to be removed or recalled.
