EVENT WAIVER
I, the undersigned, authorize the Participant named above to participate in the NWT Judo Association’s 2026 Judo Day Event.
1. I AM AWARE AND ACKNOWLEDGE that the participation in the 2026 Judo Day Event involves many inherent RISKS, which risks include but are not limited to the possibility of personal injury such as skin abrasions, nerve, bone, spinal cord and neck damage, pain, paralysis, brain injuries or even death, property damage and property loss.
2. I UNDERSTAND that the above activities require a minimum level of fitness and physical, mental and emotional health. I further understand that the probability of the risks occurring depends in part on the Participant’s level of fitness and health as well as on that awareness, care and skill with which the Participant conducts themself in the program.
3. I WARRANT that the Participant is physically, mentally and emotionally fit to participate in the 2026 Judo Day Event.
4. I UNDERSTAND, AGREE AND ACKNOWLEDGE that:
a) By choosing to participate in the 2026 Judo Day Event brings with it the assumption of the risk and I ASSUME FULL RESPONSIBILITY relative to those risks.
b) I am free to withdraw from the 2026 Judo Day Event at any time. I agree to withdraw the Participant from the 2026 Judo Day Event immediately if the Participant begins to experience any signs of transient lightheadedness, fainting, chest discomfort, leg cramps, nausea or other similar ailments.
c) The NWT Judo Association is not responsible for administering medication to the Participant for providing any medical treatment on an emergency basis or any other. If the Participant takes medication it is my responsibility to see that they do so.
Personal Information and Image Release
5. I AUTHORIZE NWT Judo to collect and use personal information about me for the purposes described in the Privacy Policy.
6. In addition to the purposes described in the Privacy Policy, I authorize the NWT Judo Association to:
a) Distribute my information to Judo Canada.
b) Photograph and/or record my image and/or voice and use this material to promote the sport through social media and traditional media such as newsletters, websites, television, film, radio, print and/or display form.
c) I understand that I waive any claim to remuneration for use of audio/visual materials used for these purposes
