To be completed by parents if athlete is a minor : In consideration of the acceptance of my registration and permission to participate in this event, I hereby release, acquit by deed and forever discharge, and agree to indemnify and save harmless Judo Canada and Judo Québec, its partners, their respective officers, executives, agents, and representatives from all claims, legal actions, costs, expenses and demands respectively to my death, injury or damage to my person or property, caused in any manner whatsoever, arising out of or by reason of my participation in this event, notwithstanding that the same may have been caused, contributed to or occasioned by negligence, breach of contract, breach of duty as occupier of the premises, or otherwise, or by the Releasees or any of them. I agree to assume all risks, known and unknown, and all consequences arising out of or in connection with my participation in this event. I agree to abide by all rules, regulations and conditions of this event. I further agree that Judo Québec has the right to use, in any form and for any period of time selected at the sole discretion of Judo Québec, without payment of any fee, photographs, images, portraits, videos or any other recordings or reproductions of me, in order to promote the objectives of Judo Québec. This includes, without limiting the generality of the foregoing, use for Judo Québec programs, events, promotional material, publications, web pages, social media, other printed material and multimedia. I, my heirs, executors, administrators, assigns and personal representatives shall be bound by this document. I have read this document and understand that by voluntarily registering for the event and placing a check mark, I am waiving important rights. If the athlete is under 18 years of age, this form has been completed by the parent/legal guardian.
